Abstract

Detoxification is one of the Ayurvedic concepts most prone to both overstatement and dismissal in modern biomedical conversation. The classical Ayurvedic detoxification framework - Shodhana therapies including Panchakarma, oleation-based protocols, bowel-support interventions, and specific hepatoprotective and antioxidant herbal applications carries a different operational meaning than the loosely-defined 'detox' marketing language that has colonised much of the wellness space. The classical framework has specific clinical indications and specific biochemical effects that modern research has begun to characterise with reasonable rigour. We consolidated 74 peer-reviewed studies published between 2014 and 2024 evaluating molecular-level evidence for Ayurvedic detoxification interventions. Studies were grouped into five frames: D1 (Panchakarma biochemistry), D2 (hepatoprotective mechanisms), D3 (lymphatic / oleation-based mechanisms), D4 (oxidative-stress clearance pathways), and D5 (gut-based detoxification). Each frame was scored on the Ayurvedic Detoxification Molecular Index (ADMI), a weighted composite across mechanistic evidence, biomarker response, clinical outcome, reproducibility, and safety. Hepatoprotective mechanisms (D2) and gut-based detoxification (D5) scored highest at ADMI 0.804 and 0.782. Our reading is that a subset of Ayurvedic detoxification interventions has real molecular-level evidence that would support their inclusion in evidence-based integrative-medicine practice, while other subsets remain under-characterised and deserve more careful investigation before being treated as validated.
