Abstract

Controlled release technologies integrated into implantable medical systems have evolved from simple diffusion-based polymer coatings to sophisticated multi-mechanism platforms incorporating osmotic pumping, erosion-mediated release, MEMS-actuated micro-reservoirs, and electronically triggered on-demand delivery. These technologies enable implantable devices to serve dual functions -- structural support or physiological monitoring combined with localised drug delivery -- creating combination products that address both the mechanical and pharmacological dimensions of disease management. This study presents the Controlled Release in Implantable Systems Framework (CRISF), evaluating five release technology platforms -- polymer matrix diffusion, osmotic pump systems, MEMS micro-reservoir arrays, electrochemically triggered conductive coatings, and AI-optimised adaptive release platforms -- across four implantable system categories: orthopaedic fixation devices, cardiovascular implants, neural prostheses, and intraocular devices. Our Implantable Release Technology Score (IRTS) integrates release rate precision, duration achievement, payload capacity, biocompatibility preservation, and system miniaturisation. AI-optimised adaptive release achieved the highest IRTS (0.924) through reinforcement learning algorithms that adjusted MEMS valve actuation timing based on biosensor feedback, achieving 96% concordance between therapeutic demand and actual drug delivery over 180 days, while osmotic pump systems achieved the highest release rate constancy (0.958) through zero-order osmotic-driven flow maintaining coefficient of variation below 4% across 12-month delivery periods.
