Abstract

Hospital-acquired infections kill tens of thousands of patients every year in Europe alone, and the relentless rise of antibiotic-resistant bacteria has made the problem harder to manage with drugs alone. Nanomaterial-based antimicrobial devices offer a different angle of attack: rather than treating infections after they start, these devices aim to prevent microbial colonisation on surfaces that come into contact with vulnerable tissue -- catheters, wound dressings, implant coatings, and surgical meshes. Silver nanoparticles have been the workhorse of this field for over a decade, but concerns about cytotoxicity at higher loadings and silver ion accumulation have pushed researchers toward alternatives like copper oxide, zinc oxide, graphene derivatives, and chitosan-based nanocomposites. The question for anyone designing or procuring these devices is which nanomaterial platform actually delivers the best balance of antimicrobial potency, host safety, durability, and manufacturability. We examined 200 nanomaterial antimicrobial device programmes across five material classes, collected from laboratories in Spain, Italy, and France between 2019 and 2023, and built a Nanomaterial Antimicrobial Device Quality Index (NADQI) that combines bactericidal efficacy, cytocompatibility margin, coating durability, elution control, and scalability readiness into a single weighted score. NADQI predicted successful clinical translation with r = +0.84 and AUC = 0.883. Silver nanoparticle programmes still led the pack (mean NADQI 0.818), but copper oxide was close behind at 0.774, while chitosan composites trailed at 0.601. Only 35.0 percent of programmes exceeded NADQI of 0.75
